I understand that policy server can download various rules on a packet gateway (PGW/ GGSN) etc but how is going to differentiate between flows from a diabetes monitor versus say smartphone. I guess that part of intelligence again has to come from either gateway or external DPI box or the application function.

I have not seen any policy server doing charging function as mentioned for VoIP, that resides on either OCS platform or IN.
