You are missing big ones:
Win 8 -for what it is
Microsoft Surface - for alienating all the partners
ipad mini - for not aiming higher (price and features) - granted this is only a blunder for Apple's way of thinking
Nokia 920 - heavy and thick like it's 2008
A bunch of carriers and iSPs- for everything they do
Facebook - for the many decisions that annoyed users
Google Nexus 4 - a great deal that can't be bought
Google - no microSD in Nexus devices ,Google drive with minimal storage, youtube search revamp, image search revamp , a "me too "music service - a general shift in ideology and a surprising lack of ideas
iphone 5 - using the wrong solution for the back of the phone - biggest blunder by revenue
RIM- delaying and delaying BB10
AMD - pricing it's new 28nm GPUs way too high at first ,loosing good will and failing to gain share when they had the opportunity
Amazon - going with a dual core A9 in the new Kindle
Asus - not getting carrier deals for the Padfone 2
moist tablet makers - for putting the breaks on Android to wait for Win RT, only to figure out much later that Win RT is not much of a solution due to pricing and the nonexistent ecosystem
NAND makers- for creating oversupply and crashing prices -depends on perspective, could be seen as a great thing
the US congress - SOPA and PIPA
Intel -for faking a live demo at CES ( was a recording playing in VLC)
Qualcomm and Nvidia - both got hit by 28nm shortages, something they should have planned better for
